Acknowledgement

By registering, I unequivocally acknowledge and comprehend that engaging in fitness and/or athletic classes and associated activities inherently involves risks, encompassing but not limited to physical injury, strain, discomfort, and the potential for serious injury or fatality. I willingly assume all risks and accept full responsibility for any injuries or medical incidents that may arise. It is incumbent upon me to communicate any physical or psychological concerns that might hinder my or my child/ren's participation in the registered activity. I confirm that I, or the child/ren I am registering, possess the physical and mental capability to undertake the registered activity at The Fusion Muslim Community Center of North Jersey. By registering, I further recognize these statements as undeniable truths and, on behalf of myself and anyone authorized to act on my behalf, agree to completely indemnify, hold harmless, discharge, waive, and release The Fusion Muslim Community Center of North Jersey, its officers, employees, agents, representatives, associates, and organizers from any liability for damages (both economic and non-economic) resulting from participation in the activity, the program, use of equipment, and/or facilities. I affirm that I, and all participants I am registering, will adhere to the rules set forth by The Fusion Muslim Community Center of North Jersey concerning the activity, use of facilities, and equipment. I commit to conducting myself in a controlled and reasonable manner at all times and refraining from using any equipment, training, or property in a manner inconsistent with its intended design and purpose. As the registrant, I acknowledge and agree to these terms for all participants in the activity at Fusion. In addition to the aforementioned risks and liabilities, I expressly acknowledge that this waiver encompasses unforeseen circumstances and events not explicitly outlined herein. I understand and agree that this waiver is intended to be as broad and inclusive as permitted by law.
