I understand that there are risks associated with my or my children’s participation in this Activity, such as physical and/or psychological injury, pain, suffering, illness, disfigurement, temporary or permanent disability, death or economic loss. These injuries or outcomes may arise from my own or other’s actions, inactions, or negligence, or the condition of the Activity location (s) or facility (ies). Nonetheless, I assume all risks of my and my children’s participation in these Activity, whether known or unknown to me, including travel to and from the Activity (including air travel) or any events incidental to this Activity.” I will not hold The Fusion Community Center of North Jersey, its management, staff, teachers, or volunteers liable for any injury or loss incurred as a result of any activity performed at the Center MEDICAL DISCLAIMER Always consult your physician before beginning any exercise program. WHEN TO CONTACT YOUR PHYSICIAN If you experience any symptoms of weakness, unsteadiness, light-headedness or dizziness, chest pain or pressure, nausea, or shortness of breath. Mild soreness after exercise may be experienced after beginning a new exercise. Contact your physician if the soreness does not improve after 2-3 days.
