Acknowledgment, Waiver, and Release of Liability

1. Adherence to Masjid Rules: By registering for this program, I agree to abide by the rules, regulations, and code of conduct of the Islamic Community Center of Laurel (ICCL). I understand that the Masjid reserves the right to ask any participant to leave the premises or terminate their participation in the program if they fail to comply with these rules or the instructions of the staff and volunteers.

2. Assumption of Risk: I understand that participation in Masjid programs may involve certain risks. I voluntarily assume all risks associated with participation, including but not limited to, accidents, illness, or injuries that may occur during the program or while on the ICCL premises.

3. Release of Liability: I hereby release, waive, and discharge the Islamic Community Center of Laurel (ICCL), its Board of Directors, employees, volunteers, and agents from any and all liability, claims, or demands for personal injury, property damage, or loss of any kind arising out of or related to my (or my child’s) participation in this program, whether caused by negligence or otherwise.

4. Personal Property: I acknowledge that ICCL is not responsible for the loss, theft, or damage of personal belongings brought to the facility.

5. Emergency Authorization: In the event of a medical emergency, I authorize ICCL staff to seek necessary medical treatment for me (or my child) if I am unable to be reached.
