Acknowledgement

By registering, you acknowledge that you will follow all the instructions provided to you by the Masjid for your and other community member's safety. I am choosing to enroll myself and/or another individual in the program at McKinney Islamic Association (MIA). I hereby release and hold harmless MIA and all volunteers and staff associated with the program, from all liability, including death, as well as all property damage or loss arising out of my/my family member’s participation in this program. I understand that this release and indemnification releases liability for the conduct of MIA, and its officers, employees, agents, representatives, and volunteers as well as the hired staff of the program. I also agree that I and my child/family members attending the program will not cause any damage to McKinney Masjid property. If any damage is caused by myself or my attending child/family members, I agree that I will be responsible for all costs of repair.
