SOCCER LEAGUE PARTICIPATION WAIVER AND RELEASE OF LIABILITY
For Participants and Guardians of Minor Participants

This Waiver and Release of Liability ("Waiver") is executed by the undersigned participant (or the parent/legal guardian if the participant is under 18 years of age), in favor of [SOCCER LEAGUE NAME] ("League Organizers") and the Memphis Islamic Center ("MIC"), including its officers, directors, agents, volunteers, employees, and representatives.

1. Acknowledgement of Risk
I understand that participation in a soccer league involves physical activity that carries with it inherent risks of injury, including but not limited to sprains, fractures, concussions, collisions, weather-related incidents, and other potentially serious or life-threatening injuries. I understand that all participants engage in this activity voluntarily and at their own risk.

2. Waiver and Release of Liability
In consideration for being allowed to participate in the soccer league, I hereby release, waive, and discharge the League Organizers and the Memphis Islamic Center from any and all liability, claims, demands, actions, or causes of action for any injury, loss, or damage resulting from participation in the league or from the use of MIC’s property or facilities, whether caused by negligence or otherwise, to the fullest extent permitted by law.

3. Indemnification
I agree to indemnify and hold harmless the League Organizers and the Memphis Islamic Center from and against any and all claims, damages, liabilities, and expenses (including attorney’s fees) arising out of or resulting from my participation in the soccer league.

4. Medical Treatment
I authorize the League Organizers or MIC staff to secure medical treatment in the event of injury or illness. I understand that I am solely responsible for the cost of such treatment and for carrying appropriate medical insurance.

5. Use of Likeness
I grant permission for the League Organizers and the Memphis Islamic Center to use my (or my child’s) image, likeness, voice, or appearance in photographs, video recordings, or promotional materials without compensation or prior notice.

6. Compliance with Rules
I agree to abide by all rules and guidelines set forth by the League Organizers and the Memphis Islamic Center, including but not limited to safety instructions, equipment use, and conduct expectations.

7. Venue Release
I understand that the Memphis Islamic Center is only the host venue for the soccer league and does not control the operation or administration of the league. I agree not to hold MIC liable for any incidents arising from league activities.
