Acknowledgement

By registering , i acknowledge that i have fully read and agreed to below waiver and will  follow all the instructions provided by  ICLI for my and other community member's safety.

                   WAIVER OF LIABILITY FOR ATHLETIC & EVENTS CENTER/FACILITY USE

In consideration of the services of the Islamic Center of Long Island (ICLI), its agents, officers, affiliates, employees, volunteers, participants and any other person acting on its behalf (collectively referred to herein as "ICLI"), I hereby agree to release, indemnify and discharge ICLI on behalf of myself, my spouse, my children and my parents as follows:

I/We hereby understand and acknowledge that the programs, activities and events held by ICLI may expose me to inherent risks, including accidents, physical or emotional injury, or illness. I/We understand that such risks cannot be eliminated and my participation in such activities is purely voluntary. I/We elect to participate in such activities despite such risks and to bear the costs of any medical treatment resulting from participation in activities. 

 I/We expressly agree to assume all risk associated with participation including, but not limited to, falls, contact with other participants, hazards, the effects of the weather, including high heat and/or humidity, and all other such risks being known or unknown and appreciated by me when using ICLI’s facilities. 

 I/We hereby acknowledge my responsibility in communicating any physical and psychological concerns that might conflict with participation in any activity.

 I/We acknowledge that I am physically fit and mentally capable of engaging and participating in the physical activity I choose to participate in.

 I/We agree to comply with all rules imposed by ICLI regarding the use of the facilities and equipment.  I/We expressly agree to not bring any type of weapon onto ICLI premises at any time and that bringing any such weapons to ICLI property will result in the immediate contact of law enforcement and prosecution to the fullest extent of the law. I/We agree to abide by ICLI's code of conduct and to act in a controlled and reasonable manner at all times and refrain from using ICLI facilities in a disruptive and inappropriate manner.  

I/We agree that if ICLI is required to incur attorney's fees and costs to enforce this waiver, I/We agree to indemnify and hold ICLI harmless for such fees and costs.

 I/We understand that ICLI is not responsible for property that is lost, stolen or damaged while in, on, or about the premises. 

 After having read this waiver and knowing these facts, and in consideration of the use of ICLI’s gym/facilities, I agree, for myself and anyone entitled to act on my behalf, to indemnify, HOLD HARMLESS, WAIVE AND RELEASE ICLI, its officers, agents, employees, organizers, representatives, and successors from any responsibility, liabilities, demands, or claims of any kind arising out of my participation or in connection with my participation in ICLI’s programs and/or events. 

BY REGISTERING I/WE INDICATE THAT I/WE HAVE READ AND UNDERSTAND THIS WAIVER OF LIABILITY. I AM AWARE THAT THIS IS A WAIVER AND A RELEASE OF LIABILITY AND I VOLUNTARILY AGREE TO ITS TERMS.
