Acknowledgement

I hereby grant permission for my child to use all of the play equipment and participate in all of the activities at Islamic Center of Wheaton (ICW). I grant permission for my child to leave the ICW premises under the watchful supervision of ICW to go on neighborhood walks. I grant permission to ICW to make whatever emergency measures are judged necessary for the care and protection of my child while under their supervision. In case of a medical emergency, I understand that my child may be transported to the nearest medical facility by ICW staff, or by ambulance for treatment, and if the arriving EMT/Paramedics deem it necessary. I give permission for ICW to take group photos to post on various ICW social media outlets. In the event of an accidental ingestion, I understand that ICW will contact the Poison Control Center. I grant permission to ICW to follow whatever directions are given by Poison Control. I hereby authorize ICW to act on my behalf in case of an emergency. ICW will take all reasonable precautions for the safety, health and well being of all students. I hereby consent to the use of photographs or video recordings taken during the course of the program for publicity, promotional and/or educational purposes. By signing my name is below, I agree to these terms. I consent to never hold ICW, Itqan Academy, nor any of its teachers, staff or students responsible in any way for any type of physical, financial, emotional or mental harm of any kind. I hereby absolve Itqan Academy, ICW, and all their affiliates from any responsibility or liability due to accident, sickness, or injury of my child.
