Acknowledgement

Authorization of Consent to Treatment of a Minor:

I/We, the undersigned, parent(s) of your children at MAS center Sunday School, a minor, do hereby authorize MAS Sunday school, teachers and school administration, to take any necessary action in the event of an emergency. Please fil out the allergies list and provide any necessary emergency devices like epi-pens to staff. 

Enrollment Policy:

The Sunday school reserves the right in its sole discretion to dismiss the above named student from school and when his/her presence at the school is judged to be detrimental to the welfare of the school, if and when he/she fails to maintain 60% attendance at the school. MAS  Sunday School reserves the right to photograph students and use photos on the school’s flyers and website.

Payment Policy:

Tuition is applicable for one semester of the academic year and is due in full upon registration in August and January. MAS Sunday School financial assistance is limited and is on a first come first serve basis till the funds are exhausted. Student(s) must meet the conditions of financial hardship to apply for assistance. If tuition is not paid on time, there will be a late fee and student(s) may be dropped.

Liability Release

By signing this contract I/we give permission for the student named above to take part in all school activities, including sports and field trips, and I/we waive and release MAS Sunday School or any of its officers, agents, or employees from all claims of liability for any injury incurred by the student at school or during any school activity.

I/We, also agree to save and hold harmless MCYC, MAS Sacramento, and their respective departments, organizations, boards, commissions, and employees from any liability whatsoever for any harm, personal injury, or property damage which my child or I may suffer arising out of his/her participation in the Sunday School program. This release is effective until revoked in writing and delivered to the principal of Sunday School at MAS center.

I/we accept and support the school’s mission, school policies, standards of discipline and the rules of behavior adopted by the school. I/we understand that it is ,y/our responsibility to notify the school office promptly of any change in my/our address and telephone number.

I/we have read , understand, and agree to all the terms and conditions of this enrollment contract. By signing this contract, I/we represent and warrant that I/we have full authority to sign this contract. I/we are fully authorized to enter into this agreement.

MCYC Community Library Student Membership:

A Sunday School student subscription to the MCYC Community library will be created for use during each enrolled semester. During Sunday School library hours, students can check-out books to take home and share with their families!

You agree to ensure your child is responsible for the library books checked out and that they be returned on time. If a book is not returned on time, a new book cannot be checked out. If a book is lost or damaged, it can either be replaced or the fees to purchase the same book can be provided.

Accounts will be created the first two weeks of school. Please look out for additional details sent home with your child.
